What is the Chronic Disease Self Management Course?
This course is a Queensland Health Sharing Health Care Project which is conducted within the Green Apple as well as in Community Health Centres. It has been designed to help people cope with chronic conditions.
This course not only deals with the physical aspects of these conditions but also talks about how to cope with the mental pain some people may feel. Developing coping strategies using proactive action planning is the key to each participant’s success.
Who will benefit from this course?
You will benefit from this course if you or someone you know has:
- osteoporosis
- arthritis (rheumatoid or osteo)
- fibromyalgia
- chronic back problems
- chronic pain
- migraines
- chronic fatigue
- heart conditions
- asthma
- cancer
- any other condition that needs self management skills

If you have had a chronic condition for more than six months you may be eligible for a lifestyle management plan (Enhanced Primary Care) through your doctor.
