Referrals
The Green Apple Wellness Centre provides special program for people with chronic conditions eg. Diabetes, Osteoporosis, Heart Conditions, Arthtritis and Asthma.

There are several ways that you could be referred or recommended to the Green Apple Wellness Centre, and a variety of Wellness Services that would be beneficial for you. These may include an appointment with an Exercise Physiologist, a Remedial Massage, or enrolment in a strength program or a falls prevention program.
Recommendation/Referral Pathways
This may come from your doctor, family or friend who knows about the Green Apple Wellness Centre’s success. You may be eligible for a Medicare funded Primary Health Care Program (usually bulk-billed sessions with the Exercise Physiologist) or for Privates Health Fund Rebates.

Medicare Enhanced Primary Care (EPC) Referral
To be eligible for a Medicare Bulk Billed Enhanced Primary Care Exercise Physiologist Referral you must present with a chronic medical condition and complex care needs. A chronic condition is one that has been or is likely to be present for six or more months. Complex care needs are ongoing and care is needed from a multidisciplinary team consisting of the referring GP. A referral must come from your General Practitioner.
Enhanced Primary Care consists of a GP Management Plan (GPMP) (Medicare Benefits Schedule Item 721) and Team Care Arrangements (TCA) (Medicare Benefits Schedule Item 723). Patients under this scheme are entitled for up to five consultations in a calendar year. Your prescription will be geared towards your personal circumstances with either a home program or Wellness Services program. Wellness Service Program provide you with sustainable physical activity options at the Green Apple which will help you comply and adhere to your exercise long-term.

Department of Veterans Affair (DVA) Exercise Physiologist Referral
Members of the veteran community (white and gold card) are entitled to a yearly referral for unlimited sessions with an Exercise Physiologist. The delivery of clinical exercise interventions manage chronic disease and the provision of post-acute exercise rehabilitation for recovering from major illness or injury. Conditions that Exercise Physiologists can help manage include diabetes, cardiovascular disease, osteoporosis, arthritis, cancer and chronic muscular conditions. Veterans are entitled to one-on-one sessions as well as group DVA strength training.

Work Cover
Work cover patients with a referral from their Case Manager may see an Exercise Physiologist for an initial and subsequent physical conditioning session.

Type 2 Diabetes Group Education Referral
Patients with diagnosed Type Two Diabetes are eligible for a Lifestyle Modification Program bulk billed through Medicare. Those that benefit most are those who demonstrate a willingness to change, are able to contribute to group processes effectively and have a potential for self management.
Before taking part in the Group Sessions, the referred patient will have a one on one individual assessment to identify if the patient is suitable to the group education and for collating a comprehensive patient history, identifying individual goals and preparation for appropriate group services.
Patients identified in the individual assessment as appropriate for group education progress to the eight group education sessions. In small groups of between 2 and 12 persons you will learn about exercise strategies, blood glucose monitoring, food labels and modification, associated health care concerns and strategies for change.
A referral is needed from your General Practitioner, and to be eligible must have Type Two Diabetes and a GP Management Plan (Item 721 or 725) in place. These education sessions are in addition to the five individual allied health services available to eligible EPC patients.

Falls Prevention Progam
Steady Steps is a falls prevention program developed by Qld Health in conjunction with Fitness Queensland’s Active Ageing Committee. The class includes some Tai Chi, specific strength work, balance and co-ordination in a fun, supportive group setting.
