Patients who are at high risk of Type Two Diabetes based on the AUSDRISK tool, can be referred to Reset Your Life under item 701 (brief) 703 (standard) 705 (long), 707 (prolonged) or item 715 (Indigenous Adult 15-54 year old health check). This allows a type 2 diabetes risk evaluation to be undertaken. the Type Two Diabetes risk evaluation is a review of the risk factors underlying the patient's 'high risk' score as identified by the Australian Type Two Diabetes Risk Assessment Tool. it includes initiating interventions, such as referral to lifestyle modification programming to assist with the prevention of Type Two Diabetes.
The Reset Your Life programme is delivered at the Green Apple. It is a series of group education and motivation sessions supporting lifestyle change, providing information in accordance with national standards. These standards require that information is provided on:

the risks of diabetes and their relationship to lifestyle risk factors;

the importance of regular diabetes screening;

nutrition advice and education;
-
physical activity advice;
-
behavioural strategies to support the adoption and maintenance of healthy lifestyle changes;

smoking cessation and alcohol reduction advice if required; and
-
information about community resources to provide support in maintaining lifestyle change.
This "Reset Your Life" programme requires the doctor to complete a GP Referral Form for each patient referred to a LMP under an item 701, 703, 705, 707 or 715. The GP Referral Form can be downloaded from the AGPN website.
